Advance Health Directive

An Advance Health Directive is an official Form used to give your personal directions regarding your future health care and is entered into directly with your GP. 

The Advance Health Directive works alongside your Enduring Power of Attorney and is document our Team provides free of charge when you draft your Will and Enduring Power of Attorney documents with our Practice.

Like your Enduring Power of Attorney, the best time to make an Advance Health Directive is now – before any urgent health conditions arise.  If you are going to be admitted into hospital, have a chronic medical condition or have a medical condition that is likely to affect your ability to make decisions in the future, now is the time to prepare your Enduring Power of Attorney and Advance Health Directive documents.
